Registered Nurse- Office Of Community Care

This position is located within the Office Of Community Care within the Philadelphia VAMC

Duties of this position include but are not limited to :

• The primary practice setting for this role is the VA Community Care Service. The practice of the nurse in this position is based on knowledge, experience, and research and has a direct impact on Community Care (CC) patients, patient safety, and appropriate flow and follow up of our patients who receive care from providers in the community.
• This nurse works with CC staff to assure that patients receive the appropriate level of care based on patient need. Population served includes veterans requiring community level primary and specialty care services in accordance with Community Care standards and guidelines.
• The highly skilled CC RN is required to assure appropriate standards of care are provided to male and female veterans ranging from young adults, age 18 to the elderly adult, age 65 and over.
• Demonstrates the ability to work effectively with patients, families / significant others, physicians, nurses and community health care professionals to optimize care in the community in the most efficient and economical manner.
• Demonstrates a sustained level of clinical practice and leadership-to improve patient care for an aging population; adult population; as well as excellent interpersonal relationship skills in interpreting and communicating service and medical center goals, objectives, policies and procedures to staff, patients and health care teams.
• Applicant is expected to attend / complete all mandatory annual review programs, rotate to other clinical team assignments within the nurse's area of expertise, and take responsibility for attending and providing continuing education activities.
• Applicant serves as a consultant to patients, family members / significant others, physicians, nurses, health care providers, VISN 4 facilities, and other hospitals as needed.
• RN possesses the knowledge and skills to communicate and interact appropriately and courteously with all internal and external customers, maintain confidentiality of electronic, written, and / or verbal patient / employee information, and demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment.
