2+ years of experience. 50-mile radius rule. No more than a 3-month work history gap in the last year. ARDMS and BLS required.

Certifications : RDMS-AB(Copy needed); BLS(Copy needed)

SSN Required

DOB Required

Work History : Should not be more than 90 days of gap in last 1 year

References : 1 Reference in entire work history
