Registered Nurse (RN) Case Manager – Home Care

As an RN Case Manager, you will provide comprehensive, holistic home care services, including :
Perform thorough head-to-toe assessments and ongoing evaluation of patient statusProvide care coordination, disease management, and medication managementDeliver skilled wound care, catheter care / management, and ostomy educationPromote infection prevention and conduct home safety assessmentsVisit in-home patients to evaluate the quality of care and review clinical documentationParticipate in ongoing interdisciplinary assessments and case conferencesProvide services ordered by the physician in accordance with the plan of careCounsel and educate patients and families to help them understand and manage their health needsComplete timely, accurate, and detailed clinical notes, reflecting patient progress and changes in conditionCommunicate effectively with the interdisciplinary healthcare team to coordinate care and plan for discharge or transitionsWhat you’ll gain by joining Us

Graduate of an accredited school of nursingCurrent, active Registered Nurse (RN) license in the State of MinnesotaAbility to travel within the service area with reliable transportation, valid driver’s license, and proof of auto insuranceCPR certification (required)Two (2) years of current nursing experience preferredOne (1) year of case management experience preferredPreferred background in community health and a broad range of medical-surgical experienceStrong communication, critical thinking, and organizational skills, with a passion for home care and patient advocacyIf you are a dedicated RN who enjoys independence in the field, meaningful patient relationships, and a supportive team behind you, this role is an excellent opportunity to grow your career while making a lasting impact.
