We are seeking a Home Health Physical Therapist to join a highly regarded, not-for-profit healthcare system in South Carolina. This role offers independence, flexibility, and strong compensation , making it an ideal opportunity for experienced PTs looking for autonomy in their schedule while still being part of a supportive team.

Key Highlights

  • Flexible Scheduling & Autonomy

PTs manage their own schedules after the initial start-of-care is booked.

  • Ability to plan your visits around personal commitments, providing true work-life balance .
  • Diverse Patient Population

    • Treat patients with post-surgical needs, orthopedic conditions, joint replacements, COPD, and general debility in a home-based setting .

  • Visits are 30 minutes on average, evaluations are 60 minutes .
  • Compensation Structure

    • Salaried + Pay-Per-Visit (PPV) Model

  • Base salary starts at $101,400
  • Additional Pay-Per-Visit earnings
  • Minimal Weekend Work

    • On-call rotation approximately once every 10 weeks .

  • No regular weekend visits-only up to three start-of-care visits per weekend (extra $5 per visit bonus).
  • Mileage & Company Vehicles

    • Generous per-mile reimbursement

  • Company vehicles available
  • Fuel & maintenance covered ; personal use of a company car available
  • Technology & Documentation

    • Provided iPhone & laptop for easy, paperless documentation .

    Compensation & Benefits

  • Competitive Salary : $101,400

    • Who We're Looking For

  • Experienced Home Health PTs Preferred , but open to strong inpatient or outpatient PTs looking to transition into home health.
  • Highly Independent & Self-Motivated - Enjoys flexibility and autonomy while staying accountable.
  • Excellent Time Management Skills - Can effectively plan their own schedule while meeting productivity goals.
