Description :

Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, the RN Case Manager is responsible for providing skilled care and case management to patients and families.

Services are provided in accordance with the State Nurse Practice Act and accepted professional standards of practice.

The RN Case Manager utilizes nursing theory and processes in their practice.

They communicate and document observations and assessments effectively.

The role includes promoting and maintaining health through direct care, teaching, counseling, and appropriate rehabilitative measures.
