This position will be located in the ER in the emergency department.

Day in the life of a Social Worker (LCSW) :

The Medical Licensed Clinical Social worker intervenes with patients who have complex psychosocial needs or may require assistance with eligibility determination for social programs. In addition, offers immediate crisis intervention to patients and families with psychosocial needs and coordinates / facilitates the development of a discharge plan of care for high risk patient populations. This role screens and receives referrals from physicians, Case Managers, staff nurses and other members of the care team.
