Pay Rate : $2322.35 - $2544.70

Required Qualifications

Physical Therapist, Rehab

Experience : 1 year

Licenses : PT-KS

Certifications : BLS-AHA(Copy needed)

SSN Required

DOB Required

Work History : Should not be more than 90 days of gap in last 1 year
