Location : Wayne or Macomb County within Michigan

Home-based | 15–20% Local Travel

Responsibilities :

• Manage a caseload and complete member assessments in home, community, or telephonic settings.
• Provide clinical support and collaborate with the Interdisciplinary Care Team (ICT) for members with medical complexity.
• Develop, implement, and coordinate individualized care plans to ensure appropriate services and address barriers to care.
• Conduct medical attestations, clinical reviews, and ensure services are provided in the least restrictive setting.
• Support members and caregivers in accessing social, housing, educational, and other community resources.
• Serve as the primary point of contact for the ICT and coordinate with internal and external stakeholders to meet member needs.
