[job_card.job_description]Test and evaluate physicaland mental abilities of residents and guests Analyze relevant medical information to determine appropriate rehabilitationgoals Consult with rehabilitation team to choose effective activity program Coordinateoccupational therapywith other types of therapeuticactivities Maintain accurate treatment records Assist in educating guests with home exercise programs and use of equipment Participate in educationand staff development courses One years experience in long-term care or related setting preferred Graduate of an accredited Occupation Therapy Assistant Program per AOTA & ACOTE standards Initialcertification obtained from the National Board for Certification in Occupational Therapy Currently licensed and / or eligible for licensure as required in the state of Colorado. Your Role :
Your primary role is to work collaborativelywith your team to provide direct rehabilitation care in occupational therapy to residents upon physicianreferral as well as to adequately plan for resident needs.
