About the Role As a valued member of our team, you will engage with patients in their homes or Skilled Nursing Facilities, conducting assessments that can last up to one hour. This role allows you to : Perform thorough adult and geriatric assessments, including medical history review, diagnosis and treatment planning, health education, and care coordination. Administer point-of-care and lab tests as needed. Collaborate with physicians, case managers, social workers, and families to provide comprehensive care. Work closely with Primary Care Physicians on patient education and follow-up care. Document assessments and care plans according to Matrix guidelines. Competitive Pay : Your income is based on the number of assessments you complete, with potential earnings ranging from $1,800 to $11,500 per month depending on your availability and location. Skills & Experience That Shine Master's Degree or equivalent experience with NP or PA licensure. Current NP or PA licensure in your practice state with prescription authority. Board certification in relevant specialties (Adult, Family, etc.). Current BLS, ACLS, or CPR certification. 1 year of experience preferred, but new graduates are encouraged to apply! Valid driver's license and proof of automobile insurance. Strong computer skills and familiarity with health record software. Excellent verbal and written communication abilities.
