About the Role :

As a Physical Therapist (PT) in Home Health, you will play a critical role in delivering personalized rehabilitation services to patients within the comfort of their own homes. Your primary objective will be to assess, plan, and implement therapeutic interventions that improve patients' mobility, reduce pain, and enhance their overall quality of life. You will collaborate closely with patients, families, and interdisciplinary healthcare teams to develop individualized care plans that address specific functional goals. This role requires adaptability to diverse home environments and the ability to educate patients and caregivers on exercises and safety measures. Ultimately, your work will empower patients to regain independence and prevent hospital readmissions through effective physical therapy treatments in a home setting.

Minimum Qualifications :

• Licensed Physical Therapist (PT) in the state(s) of practice within the United States.
• Graduation from an accredited physical therapy program with a Doctor of Physical Therapy (DPT) or equivalent degree.
• Current CPR / BLS certification.
• Proven experience or clinical rotations in physical therapy, preferably including home health or outpatient settings.
• Strong knowledge of anatomy, physiology, kinesiology, and therapeutic modalities.

Additionally, proficiency with technology, including EMR systems, will enhance your ability to track patient progress and communicate updates promptly.
