Job Description :

Play a vital role in supporting individuals as they navigate complex healthcare and social service systems. This position focuses on reducing barriers to care by providing personalized education, health system navigation, and coordination of community resources. Through a trauma-informed and person-centered approach, the Navigator helps individuals build health self-management skills, improve access to appropriate services, and address key social determinants of health. This role directly contributes to continuity of care, enhanced treatment adherence, and improved long-term health outcomes of patients living with SMI / SPMI.

Experience :

Experience navigating healthcare systems and community resources preferred.

Education :

A minimum of a bachelor's degree in psychology, social work, or a related field is required.
