Simple Solutions Psychotherapy - Founded June of 2020. We are a community-based organization providing therapy (individual, family, couples, & groups), after-school programs, mental health assemblies, organizational workshops, juvenile detention centers, case management and more! We service California residents via insurance or cash pay. Services are provided via telehealth and in-person in English and Spanish Language and based on specifics of contracts.

At Simple Solutions Psychotherapy we believe "It's SIMPLE people just make it complicated." We seek to assist PsyD practicum students in gaining knowledge and experience toward assisting the community with mental health services.

About the role

The Licensed Vocational Nurse will perform several tasks pertaining to client care asindicated and expected through acquired contracts. These duties usually involve workingwith their clients directly. The caseload of Licensed Vocational Nurses may vary.

What you'll do

• Conduct blood pressure screenings for enrolled members.
• Support Enhanced Care Management (ECM) members with complex medical conditionsand complete medical reconciliation in collaboration with pharmacy as available.
• Engages members and supports / encourages member activation towards the achievementof health goals.
• Track behavioral health outcome measures in the web-based care management platform.
• Champions healthy lifestyle changes.
• Received, identified, and follows-up treatment and medication alerts.
• Reaches out to members to identify health outcomes and for client care tracking.
• Promoting a collaborative and effective working environment within ECM by ending inevidence-based communication strategies.
• Strong sense of diversity and inclusion for ALL clients.
• Superb verbal and written communication skills.
• Compassionate and empathetic with a strong ability to establish and build trust.
• Excellent listening skills.
• Organizational skills required to manage complex, deadline-driven paperwork.
• Proficient with Microsoft Office Suite, Adobe, or related software.
