Description :

This is a contract position for an RN specializing in Case Management.

The role operates during day shift hours from 8 : 00 AM to 4 : 30 PM.

The position requires the ability to work weekends on a rotating basis.

The unit features a diverse array of patient care needs, including medical-surgical, telemetry, intensive care, oncology, pediatrics, and maternal care.

Staffing on the unit includes social workers, registered nurses, case managers, and supportive staff.

The patient ratio is approximately 1 case manager to 25 patients.

Candidates must have experience with Meditech and HCA systems.

One year of bedside case management experience is required.

Familiarity with post-acute care is essential for this position.

Preferred candidates will have Meditech, CarePort experience, and prior HCA knowledge.

The role involves planning for the progression of care and discharge planning.

Candidates will participate in multidisciplinary rounds and engage in difficult conversations regarding discharge plans with families and medical professionals.

We seek individuals with a strong, proactive personality who can adapt to various environments.

Dress code includes black scrubs, business casual attire, and no open-toe shoes or artificial nails.

Relevant guidelines and policies will be followed on the unit.
