Classification:
This is a full-time FLSA exempt position. This position is not eligible for overtime compensation.
Responsibilities:
In a 24-hour, 7 days a week CLS/CRA program, this position involves providing care and training of individuals in a residential setting. Operates under the general supervision of the DDP.
Essential Functions:
Oversee and manage the daily operation of the assigned group home. Observation and documentation of individuals’ behaviors, medical conditions and training progress on an ongoing basis. Overseeing the routine nursing care of individuals in managed home. Training and assisting the individuals in daily living skills, social skills and emergency skills. Maintain the home in a clean, healthy environment. The work is of routine difficulty and involves varied work schedules. If the event that the home needs coverage, the LPN Home Coordinator will be responsible for working with the scheduler to obtain coverage. If coverage is not obtained, the LPN Home Coordinator will cover the home. In the event of an emergency, will be required to perform CPR, First Aid and/or Mindset. The LPN Home Coordinator must have dependable transportation.
Duties and Specific Responsibilities:
- Maintaining individual's health and safety
- Observe and examine for potential medical, mental and behavioral problems.
- Report to supervisor any problems found.
- Perform all nursing duties in accordance with the state DCH Rules and State Laws specific to GA, while adhering to all facility policies and procedures.
- Handle behavior episodes appropriately according to rules and regulations.
- Record all seizure activities, which occur during shift.
- Record all behavioral incidents on proper form and record all accidents on proper form.
- Participate in monthly and quarterly fire and severe weather drills.
- Perform routine nursing care to assigned patients, while utilizing age specific and universal precaution practices at all times.
- Supervise self-administration of medications, which occur during shift and document on medication administration reports.
- Will be knowledgeable of current medications, their purpose and side effects and be able to locate medicine information. Annually checked on knowledge of medications.
- Follow policy when disposing of bio-hazardous materials, labeling and handling of all materials.
- Actively demonstrate a working knowledge of nursing theory, techniques, principles, and practice in order to holistically care for the patient.
- Practice patient teaching/training for patients and families.
- Will frequently lift up to 100 or more pounds consistently causing a high volume of stooping, bending, lifting, pulling, and twisting.
- Train individuals in daily living skills according to ISP and skill level
- Assist and/or train individuals in daily personal hygiene, bathing, shaving, brushing teeth, combing hair and use of deodorant.
- Assist and/or train individuals in skills of home maintenance, cooking skills, and grocery shopping and money management.
- Assist and/or train individuals in selection of weather appropriate, matching clothing.
- Supervises and/or train individuals in appropriate behavior in community activities, table manners and social situations.
- Integrate into community activities
- Assists supervisor in planning appropriate recreation/leisure activities based on consumer’s ISP and needs.
- Participate in extracurricular activities such as sports, field trips, camp, picnic, etc. Some activities necessitate adjusting work schedules.
- Accompanies individuals shopping, on outings for snacks or meals and haircuts. Accompany consumer to doctor and/or dental appointment.
- Attend in-service training and staff meetings
- Maintain current nursing licensure
- Keep CPR/First Aid certification current.
- Complete minimum of 16 hours of continuing training per year per Healthcare Facility Regulations
- Attend any required in-service training.
- Attend staff meetings as scheduled
- Meet with supervisor for evaluations
- Member of the Inter-disciplinary Team
- Participate in the establishment of client service plans
- Development of individual’s outcomes for ISP by determining needs, strengths and weaknesses.
- Carry out training outcomes per individual ISP and document in individual’s file.
- Provide constructive feedback and appropriate role modeling for individuals in areas such as appearance, relationships, language, social skills and daily activities.
- Discuss individual’s needs with other staff, family members and other agencies as appropriate.
- Communicates regularly with other shifts to facilitate continuity of care of individuals.
- Observe and follow group home and agency policies
- Work flexible schedule to include coming in other than scheduled hours.
- Works in any of operative sites as requested
- Carry out scheduled activities for individuals. Offer input into activities
- Review individual files and/or logbook daily prior to beginning work. Initial each day upon completion of reading
- Prepare Meals according to menus
- Follow policy on staff eating food purchased for individuals
- Maintain all group home property properly
- Obtain all receipts and support documentation for all individual expenditures
- Ensure the safety of individuals by consistently monitoring for safety hazards
- Building maintenance, safety and sanitation
- Supervise and assist the individuals in the cleaning of the home property in and around the home
- Complete daily checklist prior to the end of the shift. Record work accomplished in logbook
- Communicates to the need for repairs and maintenance of physical facilities through the work order process
- Maintains floors daily (sweeping, vacuuming, mopping). As needed carpet shampooing, waxing, or stripping floors
- Washes and disinfects walls, woodwork and windows as deemed necessary
- Cleans disinfects garbage containers and surrounding floor and walls at least weekly or more often as needed
- Clean and disinfects with approved chemicals all bathrooms, including toilet bowls
- Change and launder bed linens at least once a week or more if necessary
- Responsible for all laundry, including kitchen towels, bath clothes and towels and clothing
- Staff supervision
- Responsible for directing the work duties of other employees in the home
- Train all employees new to work location on proper procedures for the home, including medication management and client specific expectations/needs
- Suggest or recommend employees for hire, termination or promotion
- Complete all necessary administrative paperwork regarding employees under manager’s supervision, including yearly employee evaluations, absence request approval, employee warning reports and all other forms
- Responsibility for other duties
- Responsible for other duties as assigned
- Participate in staffing as requested
- In absence of other staff, will be required to assume additional duties
- Random drug testing is conducted without individualized suspicion of a violation of company’s substance abuse policy. Selection is made by neutral criteria so that all employees eligible for testing have an equal opportunity of being tested.

- Assist in transportation of individuals to all activities, doctor and dental appointments
- Passengers seated and seat belts secured before vehicle is put into motion
- Ensure individuals are never left unattended in vehicle
Qualifications:
Active LPN license required.
Experience or training in the field of developmental disabilities, mental health, special education or social services preferred.
Evidence of a satisfactory Criminal background check and MVR.
