Weekday Pay Rate : $48.40 per point

Weekend Rate : $51.33 per point

Visit Type Productivity Values

• Start of Care : 2.2 - 2.5 Points per Visit
• Discharge : 1.0 - 1.5 Points per Visit
• Resumption of Care / Recertification : 1.5 - 1.75 Points per Visit
• Revisit : 1.0 Point per Visit

Provide independent, exceptional care to patients in their homes including comprehensive assessments and creating individualized care plans, keep up with timely and thorough documentation using HomeCare HomeBase.

Cover visits between 10am and 6pm, including weekends as required.

You know how certified home health works and how to provide excellent care in collaboration with your patients, their families, and healthcare provided. We'll be here to support you as needed.
