Overview

We are seeking a compassionate and skilled Home Health Physical Therapist to join our exceptional care team in Saint Joseph, Michigan. In this full-time role, you will provide home-based therapy services that help patients achieve greater independence and improved quality of life. You'll work within a collaborative, team-based environment that prioritizes patient outcomes and professional development.

Key Responsibilities

Provide home health physical therapy services to patients across Saint Joseph and nearby areas

Perform comprehensive assessments and evaluate patient needs and functional capabilities

Develop, implement, and regularly update individualized plans of care

Deliver therapeutic interventions to improve mobility, strength, and physical function

Collaborate with interdisciplinary teams to ensure consistent and coordinated care

Educate patients and caregivers on therapy exercises and at-home care practices

Maintain timely and accurate documentation in the EMR system

Ensure compliance with all clinical, regulatory, and safety standards

Schedule and Shift Details

Full-time, 40 hours per week

Qualifications

Required

Bachelor's or Master's degree in Physical Therapy from an accredited institution

Active Physical Therapist license in the state of Michigan

Minimum of 1 year of healthcare experience, preferably in home health

Valid driver's license, reliable transportation, and comfort with travel (~20% of workday)

Proficiency in Electronic Medical Record (EMR) systems

Strong organizational and time management skills

Experience using tablets and mobile devices for clinical documentation

Preferred

Previous home health care experience

PT license number provided upon application

Additional Information

Pre-employment drug screening required (includes medical and recreational marijuana)

Must be legally authorized to work in the United States
