A well-established physician-led group with locations throughout Long Island is currently seeking to hire a Family Nurse Practitioner. This practice has built a stellar reputation for clinical excellence patient satisfaction and multidisciplinary collaboration. 100% outpatient.

Key Responsibilities :

• Serve as a key clinical provider in the delivery of high-quality care.
• Diagnose and treat a wide range of endocrine conditions including diabetes thyroid disorders (hyperthyroidism and hypothyroidism) hormone imbalances and infertility.
• Develop and implement individualized treatment plans
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses treatment options and preventive strategies.
• Maintain thorough and accurate documentation within the Electronic Medical Record (EMR) system.
• Collaborate with referring physicians specialists and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses technologists and support staff within the practice.
• Stay compliant with HIPAA NYS DOH CMS and relevant payer guidelines.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
