Offering Sign-on Bonus $10K

Relocation Assistance : Case by Case

Incentive Bonus 17.5%

Key Responsibilities

• Lead and evaluate departmental operations, including staffing, workflows, competencies, and use of technology.
• Oversee core case management functions : care coordination, utilization review, and discharge planning.
• Ensure timely, complete, and compliant documentation.
• Allocate resources to meet productivity, quality, and patient care standards.
• Collaborate with hospital leadership and interdisciplinary teams to support continuity of care.
• Maintain compliance with organizational policies and regulatory requirements.
• Foster effective relationships with clinical and administrative stakeholders.
• Adhere to organizational mission, values, and code of conduct.
