The North Central Health District's Perinatal Health Home Visiting Program is a warm, community-centered initiative designed to bring high-quality healthcare, education, and support directly to families in the rural areas of our district. Our team works closely with expectant and new parents, helping them navigate pregnancy, birth, and early childhood with confidence. By meeting families where they are, we build meaningful relationships, reduce barriers to care, and make a real difference in maternal and child health outcomes. Job Responsibilities
Under supervision, provides professional nursing care in accordance with nurse protocols, policies, and procedures, while playing a critical role in promoting maternal and child health, reducing disparities in birth outcomes, and supporting overall community wellness. This position is based at the North Central Health District Office in Macon, GA and requires travel across several counties within the district. Candidates should reside within a reasonable commuting distance. This is an excellent opportunity to work directly with communities across the region and make a meaningful impact.
Minimum Qualifications
Associate's degree in nursing from an accredited college or university and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification.

The DPH Commissioner and agency leaders encourage all employees to engage in regular wellness activities and to make lifestyle choices that promote health and well-being.
