About the Role:


We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our growing home health team. As an LVN, you will provide professional nursing care to patients in their homes under the supervision of a Registered Nurse (RN). Your role is vital in helping patients maintain their health and independence in the comfort of their homes.

Key Responsibilities:

• Provide skilled nursing care according to the physicians plan of care.
  
  
• Administer medications and treatments as prescribed.
  
  
• Monitor patient condition and report any changes to the supervising RN or physician.
  
  
• Document all nursing services accurately and in a timely manner.
  
  
• Educate patients and families on health maintenance and care plans.
  
  
• Collaborate with interdisciplinary team members to ensure coordinated care.
  
  

Qualifications:

• Active LVN license in the state of CA
  
  
• Current CPR/BLS certification.
  
  
• Valid drivers license and reliable transportation.
  
  
• Minimum of [1 year] experience as an LVN (home health experience preferred).
