Theprimary purpose of the RN Wound Care Coordinator position is to provide woundcare prevention and treatment to residents under the direction of attendingphysicians and/or Medical Director and consistent with established wound careprotocols.
JOBDUTIES & RESPONSIBILITIES:
Administration Functions
• Implementand maintain established wound care policies and protocols. Interpret these protocols to physicians,residents, family members and others as appropriate.
• Makerecommendations to the attending physician, Medical Director, DON and QualityImprovement Consultant concerning the wound care protocols, preventive measuresand specialty care of residents.
• Participatein facility surveys made by authorized government agencies as requested by theDirector of Nursing.
Direct Care Functions
• Conferwith RN Team Leaders to identify residents at risk for wounds or those whorequire assessment of wounds and treatment interventions.
• Provideon-site assessment of residents to determine status and develop plan of care.
• Implementpreventive measures and wound care, based on resident assessment andestablished protocols.
• Provideeducational health care services. Makerecommendations to the resident concerning nutrition, hydration, positioning,prevention of skin breakdown, management of chronic conditions, etc.
• Documentwound care assessments, treatments rendered and communication with residents,families and physicians in the electronic medical record.
JOBREQUIREMENTS:
Education
• Mustpossess a Nursing Degree from an accredited college or university, or be agraduate of an approved RN diploma program.
Experience
• Musthave a minimum of one (1) year experience in a long-term care, hospital, homehealth or other health related organization.
• Musthave previous experience in wound care management.
• Certificationas a specialist in wound care management is preferred.
