Palliative Nurse Practitioner

Job Description

We are seeking a compassionate Palliative Care Nurse Practitioner to join our healthcare team.

• Conduct comprehensive assessments to address patients' physical, emotional, and psychosocial needs.
• Develop and implement individualized care plans with interdisciplinary teams.
• Manage complex symptoms, including pain and acute care interventions.
• Educate patients and families on treatment options, advance care planning, and end-of-life decisions.
• Facilitate communication among providers, patients, and families for cohesive care.
• Participate in clinical research to improve palliative practices.
• Maintain accurate documentation per HIPAA and policies.
• Coordinate resources through case management activities.

Qualifications include a Master’s in Nursing with palliative focus, active NP certification, and experience in palliative or hospice care. Proficiency in EMR systems and knowledge of medical coding are required. Strong communication skills are essential. This full-time role offers a competitive salary starting at $130,000 annually and a comprehensive benefits package.
