Overview
8 hours per week or 32 hours per month; PRN HRA NP. Conduct annual Medicare Wellness exams in the comfort of patients' homes within a 30-mile radius or via telehealth for those further away. Compensation is $145.00 for each in-home visit and $85.00 for telehealth visits.

Responsibilities
The Nurse Practitioner will perform annual health risk assessments in a residential setting or via telehealth, as authorized by the Collaborative Physician.
Key Duties Include :
Conduct comprehensive in-home health assessments to identify active and chronic health issues, and assess physical, mental, and social needs.Gather patient histories, perform examinations, and establish diagnoses.Document patient visits in compliance with NCQA standards.Measure patient vital signs as needed.Make case management referrals and coordinate with primary care providers as necessary.Communicate effectively with patients, caregivers, agency nurses, other healthcare providers, and vendors.Uphold OSHA Universal Precautions during all clinical activities.Maintain patient confidentiality at all times.Participate in required meetings, training, and committees as requested.Engage in professional development activities and stay current with licenses and certifications.In this position, you will collaborate with :

Required Knowledge, Skills, and Experience :
Active and unrestricted Nurse Practitioner license in applicable states.Board certification from ANCC, AANP, or NCCPA.Valid CPR Certification.Current enrollment in Medicare / Medicaid.Valid driver's license and a clean driving record.Proficiency with electronic health records (EHR).Preferred Knowledge, Skills, and Experience :
Experience in geriatric care.Strong teamwork and effective communication skills with diverse populations.Conditions to be Aware of :
Ability to adapt to various weather conditions and different home environments.Capability to perform a full range of motion, including lifting and transporting patients. Manual dexterity, good vision, and hearing required.Regular lifting / carrying up to 50 pounds.Willingness to travel up to 150 miles daily in urban or rural settings; may require driving.Posted Min Pay Rate :
