Responsibilities Valley Hospital Phoenix is seeking a dynamic and talented PRN Discharge Planner to join our team of compassionate, dedicated professionals.
The PRN Discharge Planner works in our Clinical Services department along with Inpatient Therapist.

Duties and Responsibilities:
- The Discharge Planner will assist the clinical team with coordination of care and discharge planning.
- Responsible for having a thorough understanding of the patient’s treatment through communication with the treatment team.
- Advocate for the patient’s access to services during treatment team meetings, communication with outpatient teams and referrals.
- Meet with patients daily, be responsible for completing referrals, SMI evaluations, staffing’s, and coordinating with outpatient providers.
- Assist the team to ensure that the patient has a safe discharge placement, medication, and outpatient providers. Discharge Planners will verify safety within the home or place of discharge.
- Discharge Planners will have knowledge of outpatient behavioral health treatment interventions, substance abuse rehabilitation, referral process for SMI services-Seriously Mentally Ill and the process of referral to ALTCS-Arizona Long Term Care, Guardianship and another other form of advocacy needed to provide excellent patient care.
- The Discharge Planner will provide patient education by facilitating psycho-education groups.
Qualifications Qualifications:
- Bachelor’s degree from an accredited college or university in social work, mental health, nursing, or related degree
- A minimum of three (3) years’ experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.
Preferred Qualifications:
- L.M.S.W., LAC, LIASC, LCSW, LMFT, LPC, or other healthcare – related licensure
- Master’s degree from an accredited college or university in social work, mental health, nursing, or related degree
