The Nurse Practitioner Specialist is a pivotal member of our clinical operations team, responsible for providing exceptional direct patient care in various environments including ChenMed medical centers, nursing homes, skilled nursing facilities, and at home. Key responsibilities include, but are not limited to, conducting geriatric assessments, taking medical histories, performing physical exams, diagnosing and treating conditions, developing nursing care plans, and providing health education. You will also participate in clinical rounds and properly document all care as per ChenMed standards.

As a Nurse Practitioner, you will work independently and collaboratively with other healthcare professionals, ensuring compliance with all guidelines and participating in quality management programs. You will act autonomously as a certified nurse practitioner for your designated patient population, always maintaining a patient-centered approach.

Key Responsibilities :

• Conduct detailed assessments of acute and non-acute clinical issues, document findings, and create comprehensive patient management plans.
• Develop care plans with an understanding of cultural backgrounds and emotional needs, acting as a patient advocate.
• Manage patient care, including writing orders, interpreting lab results, prescribing medications, and coordinating referrals.
• Participate in patient care rounds, sharing management strategies with the multidisciplinary team.
• Demonstrate immediate responsiveness to medical emergencies.
• Utilize advanced communication skills to enhance patient processes and service quality.
