While your role will vary by client, it will usually include :

Home Supervisory visits

Home consultations

New Client Intakes

Caregivers / employees supervision

Oversee all aspects of PCA Tests administration

Skilled Nursing for medically fragile clients under the GAPP Program

Assess, monitor, document and report progress of patients health and condition using required documents such as clinical progress notes

Carrying out a plan of care that best supports your client

Creating compassionate and supportive solutions to your clients unique needs by collaborating with your Home Helpers associates and your clients family

Other duties as assigned

We're a great fit for you if you :

Currently hold an LPN license (GA or MULTI-STATE)

Hold a valid driver's license, auto insurance, and a clean driving record

Have a smart phone and email account

Understand the importance of ethics and client confidentiality

Applicants must be willing to complete all necessary requirements for employment
