Home Health Physical Therapist

• Requirements :

o Must have State Physical Therapist License

o Must meet productivity expectation of 30 points per week

o 1 year of home health experience preferred, in the last 3 years

o HCHB experience preferred

o Patient Assessment : Conducts thorough evaluations to determine the patient's needs and goals.

o Treatment Plan Development : Creates individualized therapy plans, often in consultation with physicians.

o Treatment Implementation : Provides hands-on therapy, exercises, and education to patients.

o Documentation : Maintains accurate records of patient progress and interventions.

o Communication : Communicates with physicians and other healthcare team members to ensure coordinated care.

o Patient and Caregiver Education : Provides education and counseling to patients and their families about their condition and treatment.

o Supervision : May supervise physical therapist assistants, in accordance with applicable regulations, according to Indeed.

o Progress Monitoring : Regularly evaluates patient progress and adjusts treatment plans as needed

o 13 week contract with possible extensions
