As an LPN, you are central to providing safe, effective, and resident-focused care. Your critical responsibilities include:
Clinical Documentation: Maintaining accurate and thorough Daily Documentation of all patient care, observations, and treatments in the resident record.
Order Management: Precisely Transcribe physicians’ orders to MARs (Medication Administration Records) and promptly Notify pharmacy of new orders and changes to ensure seamless pharmaceutical care.
Medication Administration: Prepare and safely administer medications as ordered by the physician, strictly adhering to all facility protocols and best practices.
Patient Monitoring: Proactively observe residents for any subtle or significant changes in condition, taking vital signs, and escalating concerns promptly to the Registered Nurse or Unit Manager.
Treatment Implementation: Skillfully perform and document various treatments, wound care, and procedures within your licensed scope of practice.
Supervision: Provide guidance and direction to Certified Nursing Assistants (CNAs) to ensure high standards of direct care delivery on your unit.
Perform all other duties as assigned by the Director of Nursing or Administrator, contributing to the smooth operation of the facility.

Must possess an active and unrestricted Licensed Practical Nurse (LPN) License within the state of Wisconsin.
Demonstrated ability to work effectively and collaboratively within a team.
Previous experience in a long-term care or skilled nursing setting is preferred, but highly motivated new licensees are encouraged to apply.
