Summary

Responsibilities associated with the job are directly related to the implementation of the Administrative Entity Agreement with the Office of Developmental Programs (ODP). The Priority Planning Specialist assumes primary responsibility for the implementation surrounding waiver planning activities for the Office of Developmental Supports (ODS). Incumbent will also be accountable for the utilization related to The County Assistance Office, Bureau of Autism Services and Community HealthChoices. Additional responsibilities and guidelines associated with this job are directly related to upholding the Community of Practice tools and principals of Self-Determination by giving families and individuals accurate and reliable information regarding the ID / A system, other services systems and community resources.

Essential Duties and Responsibilities

• Coordinates with the County Assistance Office to process waiver applications, transfers, eligibility renewals, and billing resolutions.
• Collects and submits required documentation to ensure individuals maintain waiver funding and Medical Assistance.
• Maintains specific spreadsheets related to billing issues, MA closures, and financial waiver eligibility.
• Enters services and authorizes waiver funding in HCSIS.
• Completes and submits updated client information forms to the County Assistance Office.
• Attends initial assessment and resource mapping for individuals / families that have multiple needs for triage.
• Utilizes knowledge of existing resources and assists in locating new available resources.
• Create relationships with organizations in the community on an on-going basis.
• Assures that the individual and family receive accurate information regarding the various ODS services / providers.
• Reviews and approves Individual Support Plans (ISPs) as it relates to job functions.
• Assures that the individuals continue to maintain on-going eligibility with the Client Information System (CIS).
• Updates information in Home and Community Services Information System (HCSIS) pertaining to an individual’s eligibility.
• Reviews the Supports Intensity Scale (SIS) for accuracy and ensures that it is incorporated into all levels across team functions for the day-to-day operations.
• Serves as a backup for approval and / or denial of SIS requests.
• Completion of specific monthly unit report data points.
• Completes annualized re-evaluations as it relates to current job function.
• Assures that individuals and family are apprised of their rights regarding appeals process. This includes conference with the county program, mediation and Fair Hearing.
• Completes service notes for all activities performed as it relates to job functions.
• Works with stakeholders to resolve issues addressing inconsistencies with system information, individuals identify incorrectly and following proper processes and policy.  Stakeholders could include :

Education / Experience Requirements

Bachelor’s degree in social work or related field, plus 1 year of previous experience working with families in the field of intellectual disability.

Any equivalent combination of education and experience that meets the required knowledge, skills and abilities.

Certificates, Licenses, Registrations

Act 33 & 34 clearances.

Valid driver’s license and access to a reliable vehicle.
