Schedule / Availability Requirements : Ongoing coverage

Setting : Acute Care / Inpatients / Interventional Neurology Lab / OR

Duties : Procedure / Op Notes and Post Notes

Days per month : TBD

Required Procedures : Endovascular Neuro Procedures, Thrombectomies, Aneurysm

EMR : Epic

Support Staff : Inpatient Hospital = APP 24 / 7; IN Lab = RN and RT both trained in Endovascular Neuro cases; OR = normal OR contingent (RN, Surg Tech, etc)

Dealbreaker : MUST be neurosurgeon with endovascular fellowship, need someone to do surgeries

Reason for Coverage : Comprehensive Intervention Coverage, required 24 / 7 coverage for a Comprehensive Stroke Center

CredentialingInformation : Timeframe : ASAP

Requirements : Endovascular Fellowship

Credentialing with other facilities?Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? Yes

Submission Requirements

Board Certification - Board Certified [Required]

State License - NT license or IMLC [Required]

Endovascular Fellowship - [Required]

Other Certifications - BLS and DEA [Required]

Malpractice - Clean [Required]

Availability at time of name clear - Required
