HOME HEALTH REGISTERED NURSE | DETAILS AND COMPENSATION :

Location : Carmel IN 46032 – 100% On-site

(Greater Indianapolis Area)

Payrate : $55.29 / hr

Required Availability : Monday–Friday, 8am–5pm (No weekends)

HOME HEALTH REGISTERED NURSE | SUMMARY AND HIGHLIGHTS :

The Home Health Registered Nurse role focuses on care assessment, plan implementation, coordination with interdisciplinary teams, and ensuring high-quality health maintenance for individuals in community-based settings.

This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance

A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

HOME HEALTH REGISTERED NURSE | RESPONSIBILITIES :

Assess health needs and implement individualized care plans.

Observe, plan, deliver, and evaluate nursing support for individuals with acute, chronic, or long-term health needs.

Coordinate with physicians, health providers, and interdisciplinary teams to ensure continuity of care.

Oversee medication ordering, administration compliance, storage, and documentation.

Monitor changes in patient condition and promptly communicate updates to physicians.

Complete daily, monthly, quarterly, and annual nursing documentation as required.

Maintain accurate nurse’s notes and health status records.

Ensure medical equipment, supplies, and environments meet health and safety standards.

Supervise LPN / LVN staff as needed.

Schedule and follow up on health-related appointments to ensure timely care.

Communicate with inpatient teams and discharge coordinators during hospitalizations.

Provide new employee training and annual safety / health recertification.

Train unlicensed staff on patient-specific needs related to safety, health maintenance, and disease prevention.

HOME HEALTH REGISTERED NURSE | REQUIREMENTS :

Education : High School Diploma or GED (verifiable)

Attendance : Commitment to mandatory attendance during the first 90 days.

Active Indiana or compact RN license.

Home health experience preferred.

OASIS experience preferred.
