[job_card.job_description]Coordinate and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses. Perform home visits and attend periodic office meetings. Manage a daily caseload of 5-10 patients, translating to roughly 6-8 points, with an expectation of 20-25 weekly units of productivity. Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits. Be open to occasional on-call shifts, approximately 1-2 times per month. Use Pointcare / HCHB EMR systems efficiently. Understanding that this is a home health assignment, involving daily travel to patients' homes and weekly clinic meetings. Comfort with potentially long drive times between patient visits. Willingness to accept every patient on the schedule regardless of travel distance. Acceptance of the contract terms, including no guaranteed hours and the opportunity to float to another clinic if needed.
