Preferred Home Care of Florida is seeking a Home Health - Clinical Manager!

Ideal candidate will have previous Home Health experience with Medicare, Joint Commission and PDGM training. This is a growth opportunity leading to the potential Assistant Director of Nursing position.

**** Axxess System Experience is a MUST!!!! ****

Medicare Oasis proficiency within a Home Health Agency and QA experience, familiar with ICD-10 coding, and the ability to generate Plan of Care as per conditions of participation. Former Field Nurse in Home Health and/or Case Management experience.

Qualifications

• Must have excellent communication skills
• Must have knowledge of Medicare OASIS
• Must have knowledge of Medicare Discipline Frequency
• Computer Skills a must (EMR- AXXESS System Core)
• Knowledge of State, Medicare and Joint Commission guideline
• Ability to audit OASIS for documentation and Discipline Visit notes
• Must make patient care a top priority
• Must have a positive attitude and be a team player

Experience


Three (3) years Home Health Nurse. Medicare OASIS, Quality Assurance (QA).

Licenses /Registration Required

• Current Nursing License in the State of Florida
• Nursing degree from accredited school
