We are looking for a License Practical Nurse (LPN) to work as needed for our non-medical in home healthcare business.
Position Summary
Our private, non-medical in-home care agency is seeking a Licensed Practical Nurse (LPN) to support clients with clinical oversight, delegated care tasks, and wellness monitoring in a home-based setting. This role is essential to ensuring high-quality, person-centered care that promotes safety, dignity, and independence for seniors and individuals with complex needs.
Key Responsibilities
• Perform delegated clinical tasks within LPN scope of practice, including:
• Catheter care and changes (Foley and suprapubic)
• Ostomy site monitoring and maintenance
• Skin integrity checks and dressing changes
• Medication administration and compliance tracking (as permitted by agency policy)
• Conduct routine wellness assessments and document client condition
• Monitor for changes in health status and escalate concerns appropriately
• Provide training, delegation, and oversight to non-medical care aides
• Collaborate with families, care coordinators, and external providers to ensure continuity of care
• Participate in care plan development and updates
• Maintain accurate, timely, and compliant documentation
• Promote trauma-informed, dignity-centered care practices
Qualifications
• Current and active LPN license in good standing (State of Nebraska)
• Minimum of one (1) year experience in home health, long-term care, or related setting preferred
• Strong clinical judgment and documentation skills
• Ability to work independently in client homes with professionalism and compassion
• Familiarity with transitional care, justice-involved populations, or trauma-informed care is a plus
• Valid driver’s license and reliable transportation required

General Physical Demands
• Ability to stand and walk for extended periods while providing patient care in home settings.
• Frequent bending, reaching, kneeling, and assisting with mobility.
• Manual dexterity to operate medical equipment and perform nursing procedures.
Lifting & Transfer Requirements
• Must be able to occasionally lift or move up to 35 pounds without assistance (aligns with NIOSH safe patient handling guidelines).
• For patients requiring transfers or repositioning beyond 35 pounds, mechanical lifts, transfer devices, or team assistance must be used.
• Ability to assist patients with mobility, including supporting ambulation, repositioning in bed, and transferring to chairs, commodes, or vehicles.
• Must follow agency safe patient handling policies to prevent injury to self and patients.
