Salary: $95,000 - $132,000 DOE
Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.
The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
We are currently seeking a full time Director of Hospice Services to assist with managing our field hospice clinical services. This position will be responsible for:
- Coordinating and overseeing all direct and indirect patient services .
- Providing guidance and counseling to direct reports including Team Leaders, when applicable, to assist them in continually improving all aspects of hospice care services, including those provided through organization personnel.
- Assisting Team Leaders, when applicable, in managing clinical teams and planning.
- Providing help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
- Responsible for monitoring, assessing, and implementing changes to improve Consumer Assessment of Healthcare Providers and Systems Survey (CAHPS) scores.
- Responsible for all staffing levels (hire, evaluation, and termination) of organization personnel.
- Conduct staff evaluations annually or more frequently if indicated.
The qualified candidates will be an RN in Illinois with a Bachelor's degree; Master's preferred. Candidates should also have a minimum of three years of management experience in hospice. Candidates should also possess the following skills:
- Possess current knowledge of hospice regulations and Medicare Conditions of Participation.
- Demonstrated ability to effectively direct a variety of personnel and utilize mentor and motivational skills.
