Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes conduct patient home visits
Supervise team of individuals who provide hospice care to our patients
Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's hospice needs
Counsel patient and patient's family on end-of-life needs
Manage emergency situations in a prompt, precise, and professional manner
Complete medication administration and treatment needs for all assigned hospice patients
Provide nursing care in accordance with applicable regulations and Company policy
Prepare and maintain accurate and complete clinical and progress notes on a timely basis
Active RN state license
Valid drivers' license
Experience working in the hospice field
Two or more years of applicable nursing experience (supervisory/leadership experience preferred)
