Location : The Villages, FL
Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM (40 hours / week)
Contract Length : 13 weeks
On-Call : Required if requested
Travel Area : Lake and Sumter counties (primarily Clermont area)
Productivity Requirement : 30 units / week
Position Overview
We are seeking an experienced Home Health Registered Nurse (RN) to join our team in The Villages, FL. The ideal candidate will bring strong clinical skills, excellent communication abilities, and solid experience in home health, OASIS, and HCHB. This RN must be comfortable managing a variety of complex patient needs including wound care, wound vacs, IV therapy, compression wraps, chest tubes, Foley care, and G-tubes

Required :Active RN license
Home Health Care (HHC) experienceProficiency with HCHB (Homecare Homebase)Experience with OASIS documentationStrong wound care and IV skillsPreferred :BSN degree
Essential Job Functions
Assess home health or hospice patients—both stable and emergent—and identify physical, psychosocial, and environmental needs.Develop, implement, document, and evaluate Plans of Care (POC) to ensure high-quality and continuous care.Collect and integrate patient data (physical, psychological, cultural, etc.) into the POC.Maintain accurate and up-to-date patient documentation for all visits and changes in status.Follow infection control and OSHA guidelines to protect patients and staff.Educate patients and families on care, medications, diet, and safety, using age- and condition-appropriate teaching methods.Supervise and guide HHAs, STNAs, and LVNs to ensure consistent and effective care.Assess and manage patient pain using appropriate techniques.Arrange necessary referrals to outside agencies.Monitor cases for compliance with third-party payer requirements.Participate in on-call rotation as assigned.Communicate effectively with patients, families, physicians, and internal teams.Attend interdisciplinary team meetings as needed.Perform additional duties as assigned.Knowledge, Skills & Abilities
Strong patient assessment skills across varied settingsExcellent communication with patients, families, and healthcare teamsSolid knowledge of medication administrationSkilled in venipuncture, IV care, wound staging and treatment, inhalation therapy, G-tube care, catheter maintenance, and use of durable medical equipmentAbility to work independently, prioritize tasks, and manage stressful situationsMaintains required licensure, certifications, and continuing educationAbility to travel locally and work a flexible scheduleCommitment to confidentiality, professionalism, and high-quality patient careEducation
