Referrals-Clinical Support

Job Specific Functions

• Respond promptly to new referrals and inquiries, ensuring timely follow-up and exceptional customer service. Receive, evaluate and input referrals into the Electronic Medical Record system.
• Educate referring parties and referred individuals regarding the referral and admission process, as well as services offered by Hospice of Spokane and insurance hospice benefits.
• Confirms insurance and obtains authorizations from payer groups as needed.
• Collects and organizes medical records to support hospice eligibility of a six month or less prognosis.
• Obtain required verbal certification of terminal illness and team assignment.
• Prioritizes and coordinates preadmission and admission visits.
• Coordinate delivery of medical equipment or medication needed for start of care, opens pharmacy account, etc. Collaborates effectively with appropriate vendors.
• Maintains confidentiality, sensitivity, kindness and compassion to clients and their surrounding family as well as referral sources.

Essential Qualifications:

• Proficient in EMR systems and Microsoft Office.
• Strong knowledge of medical terminology.
• Minimum of one year work experience in intake, scheduling or admissions (hospice, home health preferred) and/or relevant experience working in healthcare.
• Detail oriented, ability to multi-task and excellent communication skills and customer service.

Non Exempt Hourly $25-$30/hour depending on experience
