Key Responsibilities :

Collaborate with the interdisciplinary team to assess, plan, coordinate, and evaluate resident care.

Accurately document resident conditions and nursing needs in a timely manner; report significant observations and reactions promptly.

Oversee the nursing assistant's assignments, adjusting as needed based on resident needs.

Administer treatments and document responses to medications and treatments.

Prepare, administer, and chart all medications as prescribed.

Initiate physician orders as necessary.

Communicate with families regarding medication changes and resident well-being.

Ensure care plans are routinely reviewed and updated to reflect individual resident needs.

Facilitate admissions, discharges, and transfers, maintaining high-quality customer service.

Qualifications :

Graduated from an accredited nursing program as a Licensed Practical Nurse.

Hold a current Licensed Practical Nurse license, along with any other required certifications per state regulations.
