A company is looking for a Social Supports Service Coordinator (remote).
Key Responsibilities
Field a high volume of phone calls and online chats
Conduct assessments to determine individuals' goals and preferences
Provide information, answer questions, and facilitate referrals to available resources
Required Qualifications, Training, and Education
Experience in social work, case management, or care coordination
Knowledge of aging and disabilities services or long-term care
Ability to document interactions and relevant information in computer systems
Fluency or conversational ability in Spanish is encouraged
Availability to work within the established schedule of Monday through Friday, 7 : 00 am to 6 : 00 pm (Central Time)
