Job Details

  • Cardiac Nurse must be able to staff heart and lung transplants.
  • Ability to scrub a plus.

  • Primary Source Verification of professional license and / or certification, as required for the position ran within 30 days of start updated upon expiration.
  • Temporary Licenses are NOT accepted – must have a valid permanent license.
  • Proof of vaccine series or completion and approval of client medical or religious exemption form preferred, not required.
  • Positive MMR titers ONLY from within the last 15 years; booster requirements if low titer.
  • Completed Fit Test within 1 year of start, updated annually.
  • Hepatitis B vaccination series or signed declination accepted.
  • If positive, clear chest x-ray and current TB questionnaire required; chest X-Rays valid for 5 years.
  • Positive Varicella titers ONLY from within the last 15 years; booster requirements if low titer.
  • T-Dap vaccination within the last 10 years; no declinations accepted.
  • Annual Negative PPD if 2 step PPD not done within past year.
  • Agency to provide completed I-9 or attestation that completed I-9 is on file and candidate is eligible for work.
  • Live in Ohio for 5 years or more : Ohio Fingerprints (BCI&I check) AND FCRA-Compliant National Database Criminal background check.
  • Live in Ohio less than 5 years : Ohio fingerprints (BCI&I check) AND FBI fingerprint check (National coverage must be FCRA compliant).
  • Highest level of education verified through third-party; attestations not allowed.
  • Must provide verification of employment within 5 years through third-party verification; attestations not allowed.
  • Clinical skill specific checklist required within 1 year prior to start for clinical positions.
  • Competency Exams for the relevant department with at least 80% score, completed within 1 year prior to start.
  • 80% passing score required on UHS Core Competencies including HIPAA, Joint Commission-NPSG, OSHA Topics; completed within 1 year prior to start.
  • Physical Exam completed within one year of initial start date; must state traveler is able to work without restrictions.
  • Proof of Flu Vaccine required from October-March; declination requires mask wearing and specific forms.
  • 2-Step TB test with negative reads within the last 6 months; annual requirement.
  • Resume, 2 Professional References (within last 2 years), Job Application, and UH Employment History Form required for submission.
  • Non-expired photo ID required.
  • Within 6 months of start, background checks including Social Security Trace, County searches, National Criminal Sweep, and National Sex Offender check.
  • Verification to be run separately for OIG Search within 6 months prior to start; attestations not allowed.
  • Supporting documentation for SAM / EPLS / GSA / OFAC clearance within 6 months prior to start; attestations not allowed.
  • Drug Screen within 60 days upon initial start date; instant drug screens not allowed.
  • Completion of "Non Employee Required Safety and Compliance Training" modules prior to clearance; renewal annually.
  • For Ohio residents, upload FCRA-Compliant National Database Criminal background check.
