RN Home Health - Winston Salem, NC - Local or Traveler

Protouch Staffing seeking a highly skilled and compassionate Registered Nurse (RN) to provide expert, one-on-one nursing care to patients in their homes throughout the Winston-Salem area. This role focuses on case management, patient education, and utilizing strong clinical skills to help patients recover and manage chronic conditions in a non-hospital setting. This position is open to both local and traveling RNs.

Responsibilities :

• Provide skilled nursing care (e.g., wound care, IVs) to patients in their homes.
• Perform comprehensive patient assessments (including OASIS).
• Develop and manage individualized care plans.
• Administer medications and monitor patient health status.
• Educate patients and families on self-care and disease management.
• Document all visits and patient progress accurately in the EHR.
• Coordinate care with doctors and the interdisciplinary team.
• Participate in required on-call rotations.

Qualifications (Requirements) :

13 weeks

FT available, Mon - Fri, 8am - 5pm, 40 hours per week

On-call info : on call for a week once month Friday-Thursday (call is very light not busy)- call needs are subject to change based on staff and branch needs

Travel Area : Assisted Living in Forsyth and Davidson County (Guildford for coverage)

Guaranteed Hours : 32 hours

Productivity Units : 30 units

Qualifications : HCHB, Oasis, and HH experience required

This position will require RN to make visits in ALF buildings M_F 30 units a week. reg visits SOC Recerts ROC and transfer Oasis documentation. Also will require on call coverage 1 week a mont

Homecare Homebase Software Experience - Required

OASIS Experience - Required

Current state license as a Registered Nurse - Required

CHPN certification - Preferred

Proof of current CPR certification - Required

Current driver's license - Required

Must have reliable transportation and insurance - Required

1-2 years experience as a Registered Nurse in a clinical care setting - Required

2+ years Home health experience - Required.

Education

Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required

Bachelors of Science in Nursing - Preferred
