Social Worker

We are looking for a dedicated Social Worker (LMSW) to join our clients multidisciplinary team. This role is essential for supporting patients psychosocial needs, conducting discharge planning, and coordinating care within a fast-paced hospital environment.

Key Responsibilities

• Conduct biopsychosocial assessments and plan / implement appropriate patient interventions.
• Provide ongoing supportive counseling and psychosocial interventions to patients and their families / significant others.
• Collaborate and coordinate with the patient's family, community resources, and medical providers to ensure effective treatment and aftercare planning.
• Advocate for patients with social and community agencies to secure necessary services.
• Incorporate universal screening for abuse victims (including child abuse, IPV, elder abuse, sexual assault, and human trafficking) and follow all mandated reporting and hospital intervention policies.
• Complete all required documentation accurately and in a timely fashion.
• Comply with all department and hospital policies, understanding the responsibilities of an essential worker during extreme circumstances.
