RN Care Transition Coordinator-Homecare-No Weekends / No Holidays
Position Summary
To assist and facilitate care transitions across the Middlesex Health care continuum that provides optimum patient satisfaction, excellent outcomes and staff satisfaction. The care transition coordinator performs various tasks to support field clinicians with new admissions and resumptions of care to both expedite and provide follow up with difficult dispositions for home health care.
Minimum Qualifications :
A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
A B.S.N. and three years clinical experience, one in Homecare OR a diploma in nursing with 6 years full time experience in nursing, 3 in Homecare and evidence of completion of 6 credits received within two years in community health nursing theory or 6 credits in health care management from an accredited college or university or school of nursing.
Homecare experience with a variety of patient populations is highly desired.
Certification in specialty area preferred.
Knowledgeable in Homecare regulation and Procedures may also fill this position.
