About The Position

LMSW Shift : M-F 5 x 8 Days 0830-1700 On Call / Weekend Requirement : No on call requirement; traveler can pick up weekends if desired and available / they are needed Float Requirement : Float throughout patient tower across MedSurg units as necessary LOCAL RATE WITIN 60 MILES OF FACILITY Requirements : - LMSW, MD State License - BLS - 2 years of experience : in inpatient medical social work experience, with biopsychosocial assessment, assisting patients navigate the DCF process, collaborative care, care coordination with home care and equipment following discharge, experience in rehab and long term care referrals, knowledge of community programs (AFDC, Medicaid, Food Stamps, Medication Assistance, and TANF), knowledge of protective services (adult and children protective services, emergency detention, guardianship and power of attorney)

• MUST have MEDICAL INPATIENT EXPERIENCE
• this facility requires all employees to have their flu vaccination
