Description

• Conduct thorough evaluations for patients in both inpatient and outpatient environments, crafting bespoke treatment plans tailored to individual needs
• Implement physical therapy interventions designed to enhance mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Educate patients and their families about therapeutic objectives, discharge planning, and exercises for home
• Maintain detailed documentation of evaluations, treatments, and progress as per facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification favorable
