Job Summary :

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team at Woodbine Rehabilitation and Healthcare Center in Alexandria, Virginia. As a valued member of our team, you will play a crucial role in providing high-quality patient care and ensuring the overall well-being of our residents.

Responsibilities :

• Provide direct patient care under the supervision of a licensed nurse
• Administer medications, vaccines, and treatments as prescribed by physicians
• Monitor and report patient condition to the healthcare team, including changes in vital signs and overall health
• Develop and implement individualized care plans in collaboration with the healthcare team
• Conduct routine assessments and update patient records accordingly
• Participate in patient education and counseling, as well as educate families and caregivers on patient care techniques
• Collaborate with other healthcare professionals to ensure seamless patient care
