Job Title: LVN After-Hours Triage Runner

Status: Per Diem

The LVN After-Hours Triage Runner provides compassionate, timely, and professional nursing care to home health and hospice patients during after-hours, weekends, and holidays. This position supports the clinical team by responding to patient and family needs, performing urgent visits, and ensuring continuity of care after regular business hours.

• Respond promptly to after-hours calls, triage patient needs, and determine the appropriate level of intervention.

• Conduct home visits as needed for symptom management, medication administration, wound care, or other urgent clinical needs.

• Communicate with the on-call RN Case Manager, Hospice Medical Director, or Administrator regarding patient conditions and changes.

• Document all patient interactions, assessments, and interventions accurately and timely in the electronic medical record (EMR).

• Collaborate with interdisciplinary team members to ensure continuity of care and smooth handoffs to the day team.

• Educate patients and families regarding medication use, symptom management, and emergency procedures.

• Maintain readiness to travel within the service area to address patient needs in a timely manner.

• Adhere to all company policies, procedures, and regulatory standards, ensuring compliance with state and federal guidelines.

• Participate in on-call rotation and staff meetings as required.

• Education: Graduate of an accredited Licensed Vocational Nurse (LVN) program.

• Licensure: Current, active LVN license in the State of California.

• Experience:

  • Minimum one (1) year of nursing experience preferred.

  • Home Health and Hospice experience strongly preferred.

• Certifications:

  • Current CPR (BLS) certification.

  • Valid California driver’s license and reliable transportation with current auto insurance.

• Strong clinical assessment and critical-thinking skills.

• Excellent communication and interpersonal abilities.

• Ability to work independently with minimal supervision.

• Compassionate, patient-centered approach to care.

• Proficient in EMR documentation and use of mobile devices for charting.

• Flexibility to work evenings, nights, weekends, and holidays.

• Field-based position requiring travel to patient homes.

• Exposure to varying home environments and driving conditions.

• May involve physical activity such as lifting, standing, and prolonged driving.
