Job Title: LVN After-Hours Triage Runner
Department: Clinical
Reports To: Director of Patient Care Services / Administrator / Assistant Administrator
Status: Per Diem
The LVN After-Hours Triage Runner provides compassionate, timely, and professional nursing care to home health and hospice patients during after-hours, weekends, and holidays. This position supports the clinical team by responding to patient and family needs, performing urgent visits, and ensuring continuity of care after regular business hours.
Respond promptly to after-hours calls, triage patient needs, and determine the appropriate level of intervention.
Conduct home visits as needed for symptom management, medication administration, wound care, or other urgent clinical needs.
Communicate with the on-call RN Case Manager, Hospice Medical Director, or Administrator regarding patient conditions and changes.
Document all patient interactions, assessments, and interventions accurately and timely in the electronic medical record (EMR).
Collaborate with interdisciplinary team members to ensure continuity of care and smooth handoffs to the day team.
Educate patients and families regarding medication use, symptom management, and emergency procedures.
Maintain readiness to travel within the service area to address patient needs in a timely manner.
Adhere to all company policies, procedures, and regulatory standards, ensuring compliance with state and federal guidelines.
Participate in on-call rotation and staff meetings as required.
Education: Graduate of an accredited Licensed Vocational Nurse (LVN) program.
Licensure: Current, active LVN license in the State of California.
Experience:
Minimum one (1) year of nursing experience preferred.
Home Health and Hospice experience strongly preferred.
Certifications:
Current CPR (BLS) certification.
Valid California driver’s license and reliable transportation with current auto insurance.
Strong clinical assessment and critical-thinking skills.
Excellent communication and interpersonal abilities.
Ability to work independently with minimal supervision.
Compassionate, patient-centered approach to care.
Proficient in EMR documentation and use of mobile devices for charting.
Flexibility to work evenings, nights, weekends, and holidays.
Field-based position requiring travel to patient homes.
Exposure to varying home environments and driving conditions.
May involve physical activity such as lifting, standing, and prolonged driving.
