A company is looking for a Case Manager to act as a liaison between patients and healthcare providers.
Key Responsibilities
Assess, plan, implement, coordinate, monitor, and evaluate patient care options
Serve as the primary contact for patients, primary care physicians, and other healthcare providers
Ensure effective communication and collaboration within the healthcare community
Required Qualifications
Active RN License required
Associate's Degree in Nursing
Minimum three years of clinical care experience
Ability to obtain CCM certification within 18 months of hire date
Bilingual preferred
